Creating a Dynamic Distribution Form

  1. Create the form as usual
  2. Click Edit to the right of the form

  1. Select Dynamic

  1. Select a Distribution List(s)

  1. Click OK

      Click "Form Distribution Lists" Click "+ New" Add Name Add Recipient Emails Click OK
    • Exporting Form Responses

      Click the "View Responses" button to the right of the form Click "Export"
    • Building a Form

      Click “Form Builder”, then you may build your form using the tools on the left-hand side on your screen. Changes are saved automatically, once complete click the link to under “Web Access URL” to preview. 
    • View Form Submissions

      Click the "View Responses" button to the right of the form Then you can choose to "Print", "Save to a PDF", "Share", "View", or "Delete"
    • Adding a Form

      Click “Forms”  Click “+New” Name the form and type a description (optional) Click the “+” by “Send responses to” and add an email to receive the form responses (optional) Click “Ok”
